Learning Disability Health Facilitator/Acute Liaison

Job overview

A band 6 acute liaison nurse/health facilitator job has become available in the Learning Disability Health Facilitation Team.   A band 5-6 development role would be considered. 

The Learning Disability Health Facilitation Team support people with learning disabilities to access health care (in hospitals and in the community) within West Sussex.  The successful applicant would be based in the Horsham Hospital, Horsham and covering Crawley GP surgeries and hospital.

Main duties of the job

To provide specialist learning disability knowledge, expertise and input to support access to healthcare for people with a learning disability. To ensure that people with a learning disability living in West Sussex can effectively access primary and secondary health care services as required, resulting in improved health outcomes.

Kindly note that this is an essential qualification for this job:

 Registered Nurse Qualification or allied health professional and current entry on a professional register

Evidence of ongoing training in specialist area of learning disabilities and commitment to development within qualified profession

Qualifications

Essential criteria
  • Registered Nurse Qualification or allied health professional and current entry on a professional register
  • Evidence of ongoing training in specialist area of learning disabilities and commitment to development within qualified profession
Desirable criteria
  • Knowledge to degree level or equivalent

Experience

Essential criteria
  • Directly working with people with learning disabilities and their carers. Evidence of working in a multi-disciplinary / agency setting with the individual in the centre of their care
  • Experience of working collaboratively and in partnership with a range of health and social care professionals, individuals with a learning disability and their carers to plan, develop and evaluate services.
Desirable criteria
  • Facilitating and organising training workshops, developing and using health education materials for people with a learning disability
  • Experience of working with people who have a learning disability in an acute general hospital setting and undertaking a health facilitation role.

Skills

Essential criteria
  • Ability to be self-motivated, plan and organise own workload, work independently on own initiative with limited supervision and also as part of a team.
  • High standard of written and verbal communication skills including ability to problem solve, present information, negotiate effectively, listen and be empathetic and tactful.
Desirable criteria
  • Ability to present information in more accessible formats.
