A number of community staff nurse posts, full time and part time hours, on a one year contract  is an opportunity for an enthusiastic and self-motivated registered or newly qualified General Nurse to work within City Health Care Partnership CIC, in the Bridlington or Driffield locality. 

The Neighborhood care team provides holistic quality nursing care to a dedicated client group who need support and care with Long Term Conditions management /complex wound care /palliative care. Care is delivered in patients own environment or a treatment room.

The team has a multidisciplinary approach and works closely with other local providers and other healthcare providers within the East Riding.

Main duties of the job

To deliver the prescribed care as indicated in the patient care plan and to promote best practice of the 
District Nursing Service. To undertake within scope of practice initial patient care assessments and plan 
care.

Essential criteria
  • Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration
Desirable criteria
  • Post-registration qualifications
  • Mentorship qualification

Knowledge

Essential criteria
  • Demonstrate an understanding of the role of a community nurse
  • Knowledge of legislation impacting on service delivery
  • Knowledge of legislation impacting on service delivery
  • Demonstrate an understanding of evidence based practice
  • Demonstrate an understanding of evidence based practice
  • Demonstrate an understanding of the MDT
  • Basic IT skills
  • Knowledge and skills in the assessment process

Experience
