Job overview
A Band 5 Community Nurse position is available within the Haltemprice Community Nursing and Conditions Team. This permanent role is for 37.5 hours per week, scheduled over 7 days.
The position allows for flexible working hours. The selected candidate must possess strong communication skills and be able to adapt to the changing demands of community nursing. IT literacy is also required. The role requires a car and the ability to travel between various work locations, as well as for attending training and meetings.
Main duties of the job
The Haltemprice Community Nursing Team provides comprehensive community nursing care to clients requiring long-term conditions management, complex wound care, or palliative care. Services are delivered in patients’ homes (including residential care) and designated treatment rooms. The team utilises a multidisciplinary approach and collaborates with local providers and other healthcare organisations in Hull and the East Riding.
To deliver the prescribed care as indicated in the patient care plan and to promote best practice of the District Nursing Service. To undertake within scope of practice initial patient care assessments and plan care.

- Degree in Nursing Studies / First level Registered Nurse or working towards final assessment to achieve first level registration
Desirable criteria
- Post-registration qualifications
- Mentorship qualification
Knowledge
Essential criteria
- Demonstrate an understanding of the role of a community nurse
- Knowledge of legislation impacting on service delivery
- Knowledge of the modernisation agenda
- Demonstrate an understanding of evidence based practice
- Knowledge of local initiatives
- Demonstrate an understanding of the MDT
- Basic IT skills
- Knowledge and skills in the assessment process
Desirable criteria
- Able to mentor students

Experience
