A new service is being developed on Horndon ward that will be a Nurse and Therapy led service. This new unit is part of a major re-configuration of Surgical Services that will see services redesigned to provide high quality care in the right place at the right time.

If you are looking for the next step up in your career, and feel that you have solid clinical skills combined with a thirst to learn new skills and lead the daily progress of patient care at ward level then Bulphan might just be the place for you!

The FIT(Function, Independence and Transfer) Unit is a 26 bed in-patient area for those patients who have come through the critical/ acute part of their pathway and now require Therapy and Nursing input to bring them to a point of independence ready for transfer to home or rehabilitation services.

Whilst the area will be supported by Surgical physicians and ward based junior doctors it will be the Nursing and Therapy staff leading the discharge process and signing off the final paperwork.

As part of a strong Band 6 team, led by the Band 7, you would have the opportunity to develop supportive activities such as a reminiscence gym, work with the Health and Wellbeing apprentices, a new role, to encourage patients to be ready for discharge either to home or rehabilitation.

The post holder will support the ward manager in the operational management and clinical leadership of the ward and act into the Band 7 role when required to cover leave.

The post holder must be able to lead the daily clinical team to work with complex case managers, social services, third sector parties to ensure timely and safe discharge of all patients.

The post holder will ensure continued teaching and assessment programmes are in place and that the unit takes students of various levels and disciplines.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
