Mid and South Essex NHS Foundation Trust is the only acute hospital provider in the Mid and South Essex Integrated Care System (ICS), which is a system of health and care partners working together for our local population.

Within Mid and South Essex there are 149 GP practices, operating from over 200 sites, forming 27 Primary Care Networks and one Ambulance Trust. There are also three main community and mental health service providers who work together through a community collaborative.

We collaborate with other valuable partners, including three Healthwatch organisations, nine voluntary and community sector organisations and three top tier local authorities, with seven district, borough and city councils, as well as our local universities.

 

Job overview

The post holder will be responsible for delivering and maintaining the highest possible standard of nursing care, ensuring patients receive safe, compassionate and effective care throughout their admission. Care will be delivered in accordance with the NMC Code, Trust policies and evidence‑based practice, with a clear focus on patient safety, dignity and wellbeing. The role includes assessing, planning, implementing and evaluating individualised care in collaboration with the multidisciplinary team.

The post holder will demonstrate effective organisational and time‑management skills, contributing to the smooth and efficient running of the ward. This includes prioritising workload appropriately, supporting the management of ward resources and maintaining a safe, well‑organised clinical environment that promotes high‑quality care and effective patient flow.

In addition, the post holder will contribute to a positive learning and development culture within the ward, supporting the education of students and colleagues and engaging in ongoing training and reflective practice. They will actively promote continuous professional development and support a nurturing environment that encourages learning, improvement and teamwork.

Main duties of the job

The post holder will promote effective communication across the multidisciplinary team, fostering positive working relationships and ensuring safe, collaborative practice.

They will be approachable and visible, maintaining strong, supportive communication with patients, relatives and carers through regular contact and availability.

High‑quality, patient‑centred handovers will be given and received to ensure continuity of care. The post holder will follow all relevant guidelines and written procedures, maintaining accurate, timely clinical records and documentation at all times.

Excellent communication skills will be demonstrated when supporting distressed patients and families, ensuring compassion and professionalism underpin all interactions.

The post holder will assess, plan, implement and evaluate individualised care plans, ensuring all care provided is fully documented and reviewed.

Responsibility will include ensuring medicines are administered safely and in line with legal requirements and Trust policies, alongside supporting the safe custody, storage and control of medications at ward level.

Discharge planning will commence at the point of admission to support timely patient flow and maximise effective use of ward capacity. Once appropriately trained, the post holder will also contribute to patient triage, supporting safe and efficient clinical decision‑making.
