Main duties of the job

We are delighted to be able to offer the opportunity for someone to join the paediatric long term ventilation team at UHS. 

This role offers the opportunity to provide specialist care for children and young people with long‑term ventilation needs. You will support patients during their hospital stay, coordinate multidisciplinary care, and play a key role in planning safe, effective discharges home. Managing your own caseload, you will ensure each child receives high‑quality, individualised care that supports both their clinical needs and their transition back to family life.

The role also offers the opportunity to work with the WellChild charity providing education and training to staff and the wider MDT.

Detailed job description and main responsibilities

Responsibilities:
 
Have primary responsibility for the provision of specialist nursing for a defined patient/client group, practicing autonomously as a core member of the multi-disciplinary team. 
 
Be the identified key worker for a patient/client caseload to co-ordinate / oversee therapeutic treatment and care.
 
Provide specialist advice, education and support to patients, their carers and other health care professionals. Practice clinically in agreed care settings and advise and support Divisions regarding the overall management of the service. 
 
Work across professional, departmental and Trust boundaries as required to develop and support patient-centred seamless care, with particular responsibility for leading nurse-led care initiatives.
 
Additional responsibilities as part of the well child initiative: 
To be responsible for the development, dissemination and evaluation of training programmes and resources for those caring for children LTV needs.
To pro-actively manage the educational agendas within the LTV team across both the acute and community sector.
To work in partnership with the Multi Professional Team and other Practice Educators to provide education to improve, clinical nursing and service delivery.
To promote education for LTV patients by working in a variety of settings including UHS, patient’s homes, community services, schools, outpatients and regional hospitals.

Person specification

Qualifications, knowledge and experience

Essential criteria
  • Registered nurse on relevant part of the NMC register
  • Level 7 Post Grad Cert (Min 60 Credits), Master’s Degree within relevant speciality or equivalent knowledge to be assessed through evidence of relevant speciality training, experience or portfolio
  • Specialist training / Qualification in area of practice
  • Provide evidence of leading learning in practice
  • Evidence of experience and completion of all band 6 competencies within relevant speciality
  • Specialist clinical skills and experience within specialist area
  • Evidence of change Management, leadership and innovation
  • Clinical experience at Band 6 or equivalent within relevant speciality
  • Provide evidence of supporting colleagues to promote learning in practice providing an excellent learning environment for all
  • Provide evidence of supporting / managing and supervision of a range of learnings in practice
Desirable criteria
  • Counselling / advanced communication skills qualification / training
  • Act in the role of Practice Assessor or supporting the progression of learners / supporting challenges as needed
  • Experience of nurse-led care / clinics in own practice
  • Team leadership role
  • Registered as an independent prescriber
  • Leadership Qualification / training
