Hertfordshire Partnership NHS Foundation Trust, CQC rated Outstanding Trust has an exciting opportunity to recruit a senior nurse to work within the Intensive Support Team (IST). This Team provides a rapid, time limited, community based assessment and treatment service for people with learning disability living in Hertfordshire, who challenge services and have additional mental health needs. IST works with service users in their own homes and our aim is to help prevent an unnecessary hospital admission by exploring all other alternatives with the service user and their carers and/or families.
We are looking to appoint an enthusiastic Learning Disability/ Mental Health Nurse to join the Intensive Support Team (IST) in West Hertfordshire. We have a full time, 37.5 hours a week over a 7 day rota, full training will be given to person who may be looking for the next step in their career move.
Applicants will have at least 2 years post qualification experience as a Learning Disability/ Mental Health nurse at Band 5/6 level, have community service experience or worked within a learning disability or inpatient assessment & treatment setting. Applicants should have an awareness of the health needs of this client group, have excellent communication and leadership skills & be able to work effectively within a multidisciplinary team.
Applicants should have experience in assessments, planning, implementation and evaluation of care for people with learning disability and mental health needs and or challenging behavior. You will also need to have some experience or a willingness to learn, have knowledge & understanding of risk management, the Mental Health Act, DOLS and The Transforming Care agenda.
The successful candidate will be responsible for a clinical caseload, under the supervision of the Clinical Team Manager and will share responsibility for the support and supervision of junior staff in the team.
You must have appropriate UK professional registration.
