An exciting opportunity has arisen for a number of Frailty Specialist Nurses to join ENHTs expanding Frailty service on a fixed term/secondment for 12mths. At present we provide a range of innovative services to support frail elderly patients across East and North Hertfordshire, including:
- Frailty assessment unit in ED
• A rapid access Frailty Clinic
• An advice telephone line
• Outreach to Hertfordshire care homes to proactively support patients.
The successful post holder will work on a rotation basis to provide support within both the acute setting and in a community outreach capacity, contributing to Comprehensive Geriatric Assessments for this vulnerable patient group. Work will also include making and maintaining positive links with GPs and acute, community and care home staff and to support with proactively identify those patient at risk of admission
This will require the successful candidate to work collaboratively with: Interface Geriatricians, Acute general managers, GPs, Emergency Department staff, Community Rapid Response teams, care homes and community Bed Base Units.
It is expected that this role will work flexibly across the East and North Hertfordshire locality
We are a dedicated, professional, and friendly team and would welcome an experienced, enthusiastic, compassionate and motivated nurse to join us.
The post holder will need to be able to care for a group of patients with a variety of needs and communicate with the wider multidisciplinary and administrative teams.
The post requires an ability to work flexibly during the week, to be able to deliver care to the highest standards and be able to demonstrate the following which are some of the requirements for this post.
Specialist knowledge and experience of a range of acute and community care pathways including patient discharge processes, community care options and potential barriers to timely provision.
Experience in community management of long-term care like diabetes mellitus, COPD, Heart failure, chronic renal failure, dementia care and palliative care.
Completed a NMC accredited prescribing course and experience in independent prescribing
Experience in taking bloods samples and giving drugs and injections.
A strong background on working with elderly care patients is essential.
Qualifications and experience as a team leader in similar roles will be essential.
You must have appropriate UK professional registration.
