Job overview

We are looking for passionate and skilled Band 5 Community Nurses to join our Thanet Community Nursing Teams, to be based at our brand new £10million state-of-the-art Thanet Health Hub.
 
Our new hub directly supports the Governments new 10-year plan, that places community care at the heart of our health system.
 
This plan represents one of the most significant shifts in NHS history: moving care from hospitals into neighbourhoods, from reactive treatment to proactive prevention. And here at Kent Community Health,  we're right at the forefront.
 
Opened in Autumn 2025, our new Thanet Health Hub consists of an NHS community diagnostic centre, a range of community NHS services, including community nursing, relocation of an existing GP surgery and signposting and support to access health and care services provided by voluntary sector organisations, such as Age UK.

Main duties of the job

As Community Nurses we support the nursing needs of house bound patients living in and around the areas of Thanet.  We will enable and empower patients to self-care where possible so that patients can stay safe and well at home.

We provide valuable care, supporting people to manage their long-term conditions and maximise their independence, which in turn can prevent admission into hospital or enable a quicker discharge if they are admitted. As a service, we may provide wound management, palliative and end of life care and symptom control and catheter care. We also take blood and administer injectable medication.  Our aim is to ensure that our patients have access to the care that they need, at the time they need it.

You’ll play a vital role in our multidisciplinary services; managing patients with complex needs in the community.  You will have the freedom to plan care in your own way, with time to develop your assessment and treatment skills, and access to great supervision and a clinical lead to make sure you’re on the right track.  We also have a nurturing preceptee programme and many development opportunities to support you.  Our aim is to build on collaborations so that we can continue to build a positive patient experience. Person specification

Qualifications

Essential criteria
  • You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing
  • A full, valid UK driving licence, or a full, valid driving licence issued by an EU country (not exchanged from a non-EU country licence) and the ability to travel independently are essential for this role. Should you require workplace adjustments under the Equality Act 2010, we are committed to supporting you to meet the mobility requirements of the role wherever possible.
Desirable criteria
  • Mentoring qualification equivalent.

Experience

Essential criteria
  • Recent experience of healthcare gained through education, work experience or previous employment. This role is ideal for a newly qualified graduate nurse.

Knowledge

Essential criteria
  • Knowledge of up-to-date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks.
Desirable criteria
  • Having an awareness of developments within the NHS would be an advantage.
