We are recruiting for a Trust Wide Discharge Support Nurse who shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.


The successful post holder will be an enthusiastic, dynamic individual to undertake the role of Discharge Nurse.

The Trust Discharge Team work across the Hospital site case managing complex patient pathways, the Team work across a 7 day Rota as part of a highly motivated, skilled and dedicated workforce supporting patients through a complex journey from hospital back into the community.

We welcome applicants from staff in band 5 roles looking to progress into band 6 posts with required skills and experience.
