Job overview
The post holder is required to deliver an advanced level of clinical practice within the field of SMART Virtual Hospital Working with the local and nationally agreed frameworks the Advanced Clinical Practitioner (ACP) exercises independent judgment to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of patients within the acute medical / or emergency department. The ACP’s will be part of an alternative medical workforce model having undergone generalist training meeting all academic and core clinical requirements relevant to the role.
The Band 8a ACP practices in partnership with the multidisciplinary team under the clinical supervision provided by a department Consultant and/or Specialist registrar (SpR).
Main duties of the job
Work to a high level of personal and professional autonomy, using specialist knowledge and skills to deliver independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, health promotion and safe admission and discharge of patients being cared for in the 24hrs Virtual Hospital.
Leadership and management: The ACP as a senior member of the team, will also play a pivotal role in the operational development of their specialty. They will initiate, manage and drive change within the department, innovating changes in practice for the benefit of patient care in line with current trust and local programmes. Approximately 20% of the post holder’s time will be used for these purposes. The ACP will undertake the role in accordance with organisational guidelines but have freedom to act within broad policies and protocols, in accordance with professional responsibilities and boundaries.

Work in collaboration with the clinical teams on a daily basis. Monitor and action decisions in relation to clinical areas, ensuring an appropriate nursing skill mix in relation to activity and acuity. Manage and report any risk issues to the Deputy Director of Nursing, in a timely manner.
Detailed job description and main responsibilities
Ensure patients are informed and consent is gained prior to treatment, investigation and management as appropriate.
Take responsibility for the management of patients within own caseload and supervision/development of junior members of the multi-professional team.
Is able to identify the need for appropriate diagnostic tests such as X-rays and CT scans, and interpret the results.
To identify and intervene proactively where circumstances contribute to an unsafe
environment for patients, staff or relatives and escalate these to the Care Group/Departmental Risk Lead and/or Consultant Leads for Clinical Governance.
To share knowledge and expertise, acting as an expert resource to others across primary and secondary care as appropriate.
Contribute to the development and improvement of systems and processes that facilitate patient flow.
