Job overview

You will be a core member of the Urgent Care West Kent team based at Unit G, Maidstone where you will be delivering care, both clinical and non-clinical to patients/ clients.

You’ll also be key in supporting patients with their own self-care and independence. This service is part of the Urgent Care Services which provides a two-hour response to patients in crisis at home.

Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life, or experiencing an urgent need for healthcare. This may include treating acutely unwell people that would otherwise require hospital admission, and also supports hospital discharges.

The service covers 24 hours a day 365 days a year.  This service is providing nursing care within patients’ homes and covering West Kent, the ability to drive is essential to this post. 

Under the umbrella of Urgent Care is the Home Treatment Service, which provides hospital level care at home. Part of this role would be supporting the HTS team

Main duties of the job

Your daily activities will be varied to include:

  • recording you actions accurately in patient notes
  • Basic observations
  • Personal care- washing and dressing.
  • Catheter care- changing catheter bags and changing catheters
  • Mouth care
  • Stoma care
  • Supporting fluid and nutrition
  • Calibration of blood glucose machines and taking blood glucose levels
  • MRSA swabs
  • Risk assessments such falls assessment, nutrition and hydration, skin integrity
  • Sub cut injections. (e.g.: insulin injection).
  • Venepuncture

Person specification

Qualifications, training and experience

Essential criteria
  • Qualifications and Credit Framework (QCF) Level 3. (NVQ Level 3 apprenticeship, BTEC or equivalent international qualification in healthcare; or demonstrable experience at an equivalent level)
  • Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C/ 4-9 or equivalent in English and Maths)
  • Full UK driving licence
Desirable criteria
  • Care certificate

Knowledge and skills

Essential criteria
  • Sound communication skills in order to understand and communicate both verbally and in writing.
  • Customer focussed approach which is essential to; provide patients with the best possible experience; represent the Trust when talking to and working with patients, colleagues and visitors and work together with colleagues and other teams across the Trust.
  • Knowledge of up-to-date clinical practice by using skills and adhering to evidence-based procedures and clinical effectiveness frameworks.
  • Fully understand the role and remit of a health care worker.
