Job overview

Join our Community Nursing Team and make a real difference to patients' lives by delivering high-quality nursing care to housebound adults aged 18 and over. Our service supports individuals with complex and long-term health conditions, helping them remain safe, well, and independent in their own homes. As a Nurse Associate, you will work autonomously and as part of a multidisciplinary team, providing personalised, holistic care to patients within their chosen environment.
You will undertake home visits, manage a delegated caseload, participate in team handovers, and work collaboratively with colleagues, carers, and partner agencies to ensure seamless patient care. This is a rewarding and varied role that offers the opportunity to build strong therapeutic relationships while supporting patients to achieve the best possible health outcomes.
Main duties of the job

No two days are the same in Community Nursing. You will deliver a wide range of clinical interventions and assessments, including monitoring and accurately recording vital signs such as blood pressure, respiratory rate, oxygen saturation, temperature, and Glasgow Coma Scale (GCS). You will administer medications via oral, topical, intravenous, subcutaneous, and Controlled Medicines Routes (CMR), undertake venepuncture and peripheral cannulation, and provide care and management of intravenous devices.
Additional responsibilities include male and female catheterisation, bladder and bowel care, wound assessment and dressing management, removal of wound closures and drains, symptom management, and holistic patient assessments. Working closely with the multidisciplinary team, you will contribute to care planning, communicate effectively with other agencies involved in patient care, and support the delivery of safe, compassionate, and person-centred services within the community.

Person specification
Qualifications, training and experience
Essential criteria
- You will have either: A Qualifications and Credit Framework (QCF) Level 3. This could be an NVQ Level 3 apprenticeship, BTEC or equivalent international qualification in healthcare Or you will be able to demonstrate the equivalent level of experience.
- Registered with the NMC as a Nursing Associate
- You’ll need to have recent experience of healthcare/ working as a health care assistant gained through education, work experience or previous employment
- A full, valid UK driving licence or a full driving licence issued by an EU country (not exchanged from a non-EU country)
Knowledge and skills
Essential criteria
- You will need to have a customer focussed approach which is essential to; Provide patients, clients and their families with the best possible experience
- You will need to be able to be able to use IT systems and databases
- You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a Nursing Associate.
- You will understand where to signpost clients and families to alternative services to support behaviour change and improve health
Desirable criteria
- Having an awareness of developments within the NHS would be an advantage

You must have appropriate UK professional registration.
