The Safety and Governance lead will manage the governance agenda for Women’s Services ensuring systems and processes are in place and adhered to for clinical effectiveness and patient safety.  The post holder will provide leadership, guidance and expertise within the wider multi-professional team for quality and governance. This will include putting in place strategies and frameworks that ensure services meet national, regional and local standards for good governance.

The post holder will implement and monitor the Clinical Risk Management and Safety Strategy for Women’s Services in line with the Trust’s Clinical Risk Management Strategy. They will oversee the governance team in working with clinical staff to identify risks, review risk assessments, report risks appropriately and will ensure that necessary actions are identified and completed and that learning takes place.

The person will be required to participate in the on-call provision.

Main duties of the job

•    Work closely with the Head of Maternity Governance, the senior management and directorate teams within Maternity to ensure that the service meets the statutory requirements and standards for Patient Safety Governance.
•    Responsible for leading the maternity patient safety agenda and ensure a unified approach to the review of patient safety incidents across the service, in line with the Trust Policies & Procedures and best practice guidance.
•    Be regularly visible / present in clinical areas, encouraging interaction with all groups of staff and gathering ‘soft’ data to enable triangulation of data and establish effective working relationships.
•    Work closely with the other quality governance teams within the Trust to ensure meaningful and proactive triangulation of data
•    Act as a focal point for incident reporting within Maternity and encourage all staff to make use of the In-phase reporting system and share resulting good practice and lessons learned.
•    Escalation of incidents within Maternity that meet the Trust Patient Safety Incident Response Plan to the weekly Risk Review meeting and to the Patient Safety Incident Review Group (PSIRG)

All successful applicants will be required to complete our corporate trust induction once a start date had been agreed. 

Following your corporate induction you will be expected to attend the Trust’s 2-week Induction Programme for RNs, NAs, RMs, ODPs and Paramedics. The programme is full-time (please email [email protected] if this is problematic) and will involve travel to both Trust sites. The programme includes supernumerary shifts in the clinical area, classroom-based sessions and e-learning.

Person specification

Qualifications

Essential criteria
  • Registered Midwife or medic with relevant experience
  • Masters level degree, willingness to work toward or equivalent professional experience
  • Evidence of continuous professional development
  • Root cause analysis training
Desirable criteria
  • Completed Patient Safety Incident Response Framework (PSIRF) related Health Services Safety Investigations Body (HSSIB) training
  • Risk Management Qualification or Equivalent
  • Leadership qualification

Experience/ Knowledge

Essential criteria
  • Experienced midwife with at least 5 years post registration experience
  • Previous significant midwifery experience at a senior level
  • Previous experience working in either cooperate or midwifery governance
  • Detailed knowledge of risk and governance processes
  • Experience & evidence in identifying learning from incidents and ensuring that learning is shared and embedded
  • Experience of managing Datix/inphase and supporting incident handlers to investigate and close incidents
  • Experience of managing difficult situations
  • Knowledge of the principles of Duty of Candour and experience in providing verbal and written DoC to women and families
  • Evidence of skills in diplomacy and negotiation
  • Able to demonstrate wide knowledge of health policy and application to practice and processes
  • Understanding of NHS organisation and current issues
  • Previous audit/QI experience
  • Adapt knowledge of the risk register, how it functions and the management
  • Understands the role of MNSI/PMRT
  • Knowledge and experience of PSIRF framework
Desirable criteria
  • Experience of operational management and manager on call
  • Experience of managing a project or a service development change
  • Experience with working with multidisciplinary team and other staff at all levels.
  • Practical experience of Clinical Governance principles, systems and processes with in experience of CQC
  • Experience of risk assessment
  • Evidence of staff development and training.
  • Experience of CQC Assessment standards
  • Knowledge of current research and relevance to practice
  • Ability to relate to a wide range of stakeholders
