The Medway Community Mental Health Service for Older People requires a Community Mental Health Nurse with an interest in working with older people with mental health needs, with both organic and functional conditions. This is an exciting opportunity for an enthusiastic, motivated individual to join our experienced team working full time (37.5 hours per week).

Our team provides assessments and interventions within various community settings, including home visits, clinics and primary care, engaging in multi-disciplinary and multi-agency working. There is an opportunity to further develop specialist skills as well as more generic roles such as CPA care Co-ordinating and facilitating groups.

The post holder will be based at Elizabeth House in Rainham, Kent and will be encouraged to participate in clinical forums. The purpose being to facilitate the sharing and development of skills, provide support networking and to develop evidence based practice as part of integrated clinical pathways.

As part of KMPT's attraction package, we currently offer a New Hire Bonus of up to £1200 for registered nursing staff joining in a band 5 or 6 Front Line nursing role with direct patient care.

Effective communication skills and self-governance are required. You will be able to demonstrate knowledge of high level interventions & community assessment skills. Existing qualifications or a willingness to undertake training relevant to the post is expected.
