Job overview

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

Main duties of the job

Our Community Nursing Service provides high‑quality care to housebound adults with complex and long‑term conditions. Working within teams aligned to local Primary Care Networks, we deliver integrated, proactive care that prevents avoidable hospital admissions and supports patients to achieve their long‑term health goals.

As a Band 6 Community Nursing Team Leader, you will play a key role in leading and supporting the team to deliver excellent standards of care. You will supervise and develop staff through one‑to‑ones, appraisals, clinical supervision and return‑to‑work meetings, ensuring a positive and supportive learning environment. You will act as a senior clinician, developing integrated care plans with senior practitioners and commissioners, and guiding junior staff in assessing and managing patients with complex needs.

Your role will include undertaking a range of clinical and non‑clinical interventions, prioritising caseloads, and contributing to service improvement and statutory requirements. You will work collaboratively with the wider MDT and external agencies to ensure seamless, patient‑centred care

Additional Working Pattern: 7.5 hrs a day between Monday and Friday with late and weekends in rotation

Person specification

Qualifications, training and experience

Essential criteria
  • You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing
  • You’ll need to have experience of managing complex caseloads and supporting/ mentoring others
  • You will be a registered health care professional with the Nursing and Midwifery Council.
  • A full, valid UK driving licence or a full driving licence issued by an EU country

Knowledge and skills

Essential criteria
  • You will need to have a customer focussed approach
  • Provide patients, clients and their families with the best possible experience
  • You will need to be able to be able to use IT systems and databases
  • You will understand where to signpost clients and families to alternative services to support behaviour change and improve health
  • You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker
Desirable criteria
  • Having an awareness of developments within the NHS would be an advantage as well as an understanding of National Service Frameworks and NICE guidance
