Job overview

The Rehabilitation Nurse (Band 5) is responsible for ensuring the effective provision of nursing care for patients in the unit. She/he may act up for the senior Staff Nurse and will regularly take charge of a nursing caseload when the relevant competencies have been achieved. She/he shares responsibility for the management of rehabilitation programmes for patients while on the unit. She/he carries continuing responsibility for the comprehensive assessment of care needs, the development, implementation and evaluation of care programmes. She/he is accountable for standards of care and for determining the activity of her/his support staff. You will be expected to:
1. To adhere to the NMC Code of Professional Conduct and to be conversant with the Scope of Professional Conduct and other NMC Advisory papers.
2. To take responsibility for ensuring the criteria for CPD is met in order to maintain nursing registration and also for professional and organisational knowledge in accordance with PREPP.
3. To practice according to Bromley Healthcare Policies and Procedures.
4. Demonstrate active interest in at least one key area of community nursing.
5. To act in accordance with nurse prescribing guidelines if applicable.
6. To actively participate in own appraisal and to access clinical supervision process as agreed.
7. To provide integrated patient care with other members of the multi-disciplinary team.
8. To actively participate in team meetings and disseminate

Main duties of the job

Clinical
• To provide high quality and skilled nursing/rehabilitation care and support to patients, carers and their families of a defined caseload.
• Liaise with community teams, GP surgeries, statutory and voluntary agencies in the provision of care to patients and their families/carers in order to facilitate a safe discharge.
• To undertake holistic assessments of the patient, using appropriate documentation according to Bromley Healthcare’s Policies and Procedures. This may include: falls-, continence-, wound care- and outcome measure assessments.
• To participate in multi-disciplinary activities such as case conferences, joint assessments, goal setting, team meetings and discharge planning.
• To plan, implement and evaluate evidence-based programmes of care.
• Ensure that all electronic and manual data is collected/recorded in an accurate and timely manner.
• To work and maintain community nursing standards as agreed by Bromley Healthcare and participate in education and peer audit. All staff have a responsibility to inform managers and leads when Trust quality standards cannot be met.
