Community Health Care Assistant

Band 3 - Permanent,  Part Time 2 x 15hr per week

If you are a Self –motivated, dynamic individual with a passion for community care, this could be the position for you.

You will be working alongside a multi-skilled team of community practitioners led by Specialist Practitioners in District Nursing. Your role will be to support the team with service delivery supporting with clinical interventions and providing care to vulnerable individuals.

The District Nurse team promotes wellbeing and independence to all our patients in an understandable and approachable way. You must be able to display effective communication skills with patients, family, carers as well as the wider Multi professional workforce.

Main duties of the job

The Health Care Support Worker for the District Nursing Service will assist the team in the delivery of high quality nursing care to patients in the community setting. The individual will be proficient, competent and work without direct supervision in a patient’s own home or clinic setting.

Qualifications

  • GCSE English and Maths or equivalent

  • NVQ level 3
