Job overview

An exciting opportunity has arisen to join our dynamic district nursing team within the Out of Hours District Nursing service as part of WWL. The shift pattern is Monday to Sunday with 3 night shifts (2000-0800).

We are seeking a highly motivated and forward thinking Health Care Support Worker with community experience to assist the Out of Hours District Nursing Team in the delivery of urgent high quality patient focused care to patients in their own homes.

The post holder will be required to work collaboratively with the wider integrated care teams.

The role requires excellent communication and problem solving skills. Experience of palliative and end of life care is essential to the role. The ability to work as part of a team and to have an adaptable approach to work is essential.

The service operates a 7-day service, and this role is part-time (33 hours) over 3-night shifts. The role requires an Enhanced DBS with Adults Barred List check, which will be obtained during pre-employment checks.

Full driving licence with business use on car insurance essential.

Based in the Community division so you will need to be able to travel to Ashton, Wigan and Leigh.

Candidates will be asked to provide original qualification certificates to the interview panel

 

Main duties of the job

·         To assist in the delivery of Nursing care to patients/carers in their homes or clinic setting as delegated by the team coordinator/team leader.

·         To undertake the extended role of a qualified NVQ support worker after appropriate Trust training.

·         To communicate effectively and appropriately with all team members and other agencies.

·         To undertake delegated duties which facilitate the smooth running of the district nursing team.

Detailed job description and main responsibilities

  • To undertake the extended role of a qualified NVQ Level 2 qualified practitioner after appropriate Trust training.
  • To undertake clinical procedures as delegated by the Team Coordinator/Team Leader and undertake invasive clinical procedures with set criteria, e.g. gastrostomy care, wound care insulin administration.
  • To provide advice and health promotion to patients/carers as appropriate.
  • To undertake clinical observations.
  • To obtain specimens as clinically indicated and as requested.
  • To accurately document patient care in accordance with Trust policy.
  • To inform Team Coordinator/Team Leader of changes in the patient’s condition.
  • To take delegated responsibility for maintaining stock control and secure storage of stock.

  • RQF Accredited NVQ Level 2 in Health & Social Care (or Care Certificate obtained via observation at work - not online modules)
