Children's Community Nurse - Pilgrim Hospital

NHS AfC: Band 6

An exciting opportunity has arisen for an experienced and accomplished Registered Children’s Nurse to join the Community Childrens Nursing Team based at Pilgrim Hospital, Boston.

The local teams consist of a CCN Team Leader, Band 6 CCN’s, HCSW and Administrative Support who work closely to provide a link between hospital and home and offer expert care in the child’s home.

The service is routinely available Monday-Friday 09.00-1700hrs, however team members will be required to work flexibly to provide ‘before’ and ‘after’ school visits depending upon the needs of individual children and their families.

You will participate in a 24/7 County-wide on-call system which will operate for children requiring palliative / end of life care.  There is also an expectation that should it be required, successful candidates will participate in working a shift system including weekends to support the future development of the service.

Working as part of the team and in partnership with the child, their family / carers, you will formulate individual care packages that facilitate the smooth transition from hospital to home.

You will have the responsibility of managing your own caseload and coordinating services to facilitate early discharge, prevent hospital admissions and provide direct nursing care to children and young people with acute health care needs and those with long term conditions at home.

As an excellent communicator with a high standard of teaching and presentation skills you will support the development of ambulatory care and promote the ethos of empowering children and their families to develop the knowledge, skills and expertise to enable them to become self-caring, independent and manage their own condition at home wherever possible.

Opportunities for further professional development, training and regular 1:1 support from the Team Leader will be offered. There is a focus upon maintenance of clinical skills and therefore you will work across all ULHT Children & Young People’s Nursing Services when required i.e.) within acute inpatient & outpatient services. You will undertake an allocation of 150 hours each year on the Childrens wards to ensure you have a wide range of on-going experience and clinical skills and have the ability to support all Childrens services if and when required.

Applicants must be RN (Child) / RSCN. Recent CCN and community caseload management experience would be highly advantageous as would the Community Children’s Specialist Practitioner qualification.

These challenging posts require highly motivated and innovative Children’s Nurses who are able to work independently, flexibly and are dedicated to the development of our Children and Young People’s Services in Lincolnshire.

Applicants must have the ability to travel throughout the County.
