Our vision is for the people of Croydon to experience well-co-ordinated care and support in the most appropriate setting, which is truly person-centred and helps them to maintain their independence.
As demand rises with increasing health needs and advances in clinical care, we are responding to the challenges whilst continuing to deliver the highest quality of care. We are developing the services that we currently provide to reduce hospital admissions and to facilitate earlier discharge from hospital through advanced holistic assessment and ensuring patients have the safest best care that is delivered in their home environment.
Living Independently for Everyone- LIFE
Croydon Health Services are in a new Alliance Partnership with other health and social care providers in Croydon and the LIFE service is one of our new integrated service models. This means bringing together health and social care teams who work to support people to remain independent and healthy in particular to prevent admissions to hospital and shorten hospital stays.
The Rapid Response Service is an admission avoidance service available 24 hours a day, 7 days a week.
Are you a dynamic, flexible, innovative and motivated experience District Nurse, Nurse practitioner with Community experience? Clinical expertise and good communication skills are required as well as excellent time management and prioritization skills to provide care to this group of patients living within their own home or within the Intermediate Care beds.
The post holder will require advanced assessment skills and have the ability to prescribe a plan of care that will support patients with complex needs through an acute episode in the community in order to avoid a hospital admission. The candidates will need to be motivated and innovative with strong leadership and communication skills, to support the development of community services.
