Advanced Clinical Practitioner in Frailty
Band 8a
The Acute Liaison and Frailty Assessment (ALFA) Team provides a responsive, safe effective caring service that is well led.
The team is commissioned to reflect the needs of the local community. Service aims include urgent case management, proactive case management, self-care management and adoption of a co-produced approach with patients and service users.
The post holder will spend a minimum of 90% of their time in direct autonomous clinical practice, having clinical responsibility for a cohort of patients and providing leadership for clinicians within this clinical area.

Main duties of the job
The Frailty Practitioner will join the acute frailty team to undertake holistic geriatric assessments for older patients living with frailty. Working closely with the Frailty Consultant Practitioner and the wider multidisciplinary team, the practitioner will assess and follow up patients both in and out of hospital as needed.
The role involves providing expert, evidence-based, person-centred care that prioritises safety, dignity, and privacy. Practitioners will deliver timely assessments to identify patients who require hospital admission and those whose needs can be met through Frailty Same Day Emergency Care (FSDEC) with or without primary and community input, supporting better outcomes for patients.
The role will be responsible for the line management and professional development of junior and other staff.
While the frailty team is based in the acute hospital, its success relies on timely, coordinated care across the integrated health and social care system. The Frailty Practitioner will provide expert clinical advice, leadership, and support, ensuring patient needs are met by collaborating effectively with community colleagues. This requires exercising advanced clinical judgment, critical analysis, and decision-making to deliver safe, high-quality, and responsive care across the care pathway.

Qualifications, Training
Essential criteria
- Registered Allied Health Professional/Registered Nurse
- Post registration experience gained by undertaking on-going personal development and training focused on older adults / frailty / advanced practice skills
- Prescriber (or working towards)
Desirable criteria
- Training in motivational coaching and/or interviewing
- Master’s degree level award (or working towards) relevant to the role.
Skills and Abilities
Essential criteria
- Previous experience in completing advanced care plans.
- Able to lead a team successfully.
- Commitment to partnership working, including ability to work cooperatively with GPs, colleagues and other stakeholders.
- Strong communication skills (listening, verbal and written) that enable you to inform and advise others clearly, and communicate with a variety of audiences including clients, health and care professionals, commissioners and other organisations’ staff.
- Ability to deliver services in a person-centred way (enabling/empowering approach) promoting independence in clients
- Knowledge of current community service changes and direction.
- Knowledge of clinical risk strategies and able to apply risk reduction techniques.

- Experience of managing a caseload as a senior clinician.
- Demonstrable experience of working with people with health and social care needs, or community-based work with socially excluded groups, or those experiencing health inequalities.

Desirable criteria
- Successfully implementing change including change that involved staff.
- Experience managing staff within a team, resources and workload e.g., effective use of team skill mix, appropriate delegation of patient care, supervision of junior staff.
Knowledge
Essential criteria
- A broad understanding of health and social care services.
- Understanding of how to monitor and improve service quality for the benefit of customers, and the ability to demonstrate and foster a culture of sharing best practice with colleagues and learning from others.
Desirable criteria
- Knowledge of local area, and local community and voluntary sector services.
