0-19 Service - Specialist Public Health Practitioners - Health Visitors / Band 6 vacancies
We have an exciting opportunity for Health Visitor ANS/OR School Nurse SCPHN to be part of the new integrated Public Health Nursing 0-19 Service.
The SCPHN practitioner will work as part of a Team in one of the three localities across Croydon delivering the Healthy Child Programme (HCP) to either the 0-5’s or the 5-19’s. This will include working in partnership with all the Public Health 0-19 Nursing team, service users, professional networks and stake holders to provide Commissioned Services to meet Key Performance Indicators
This is achieved through effective assessment, delegation, case/ workload management, monitoring of care standards, leadership and the performance management of team members. The practitioner will also be expected to take a lead in one of the high impact areas.
Main duties of the job
1. To work in partnership with team members, service users, Croydon Health Service (CHS) staff and all stakeholders in promoting the health and well-being of Children, Young People, and their families. This will be achieved through effective delegation, workload management, monitoring of care standards, leadership and the performance management of team members
2. To identify and monitor health needs of a given population using profiling tools and facilitate the implementation of evidence-based health care. The practitioner will work in line with Working Together, and the Healthy Child Programme and all policies that promote and safeguard children 0-19 years and their families
3. To assess client needs, using the Assessment Framework and Risk Assessment tools as appropriate and plan, implement and evaluate evidenced based care given
4. To work in partnership with all staff within the PHN 0-19 service Team Leads, Practice Educators and multidisciplinary teams in the development of innovative clinical practice in line with the Healthy Child Programme

Essential criteria
- To have a clinical qualification RGN/RN
- To be a Specialist Public Health Practitioner Health Visisting qualification at Degree or Post Graduate Level
- To have undertaken Nurse Prescribing or a commitment to undertake Nurse Prescribing training
