Job overview
Take a proactive approach to quality and safety management within Women Services Group.
Lead investigation following serious incidents. Compile and submit reports to the Trust Executive Team and when appropriate to the Clinical Commissioning Group (CCG). Advice and support staff undertaking Root Cause Analysis (RCA) investigations.
Contribute to the achievement of compliance with Care Quality Commission (CQC) Essential Standards, Maternity NHSLA, CNST Standards (Clinical Negligence Scheme for Trusts) and any other best practice guidance which include NICE and National Confidential Enquiries. Responsible for Horizon Scanning for any updates or new legislation which will impact on the Risk and Quality Framework for the department.
Presentation of cases at the Clinical Governance meeting and where appropriate within the Department so as to ensure lesson are leant following serious incidents.
In line with the aims of the Maternity Risk Management Strategy the post holder will raise the profile of best clinical risk management processes to achieve improvement with client care and reduce adverse incident.
The post is not area specific; all trust employees will work as appropriate to the needs of the service.
Main duties of the job
Lead in the identification of Women Services incidents in accordance with the triggers identified.
Where appropriate deputise for the Head of Quality and Safety for maternity
Ensure that the Director of Midwifery, Clinical Lead, Head of Quality and Safety, Trust Patient Safety and Risk Manager are informed of all Serious Incidents (SI) which require investigation in line with the CQC regulation and Trust policy.
Ensure that incidents are dealt with sensitively and in a timely fashion and where appropriate lessons are learnt and practice changed.
Ensure that risk management is embedded into the maternity culture.
Contribute to the implementation of the maternity risk strategy to enable its implementation to agreed timescales ensuring that the programme is congruent with issues identified in the course of planning for external assessments and reviews.
Implement the trust risk management strategy & policies ensuring that they are Maternity Clinical Negligence Scheme for Trusts (CNST) compliant, woman focused and regularly reviewed and amended in the light of changes, and legislative and regulatory requirements.

Detailed job description and main responsibilities
Ensure that any changes as a result of incidents are understood and implemented, encouraging reporting and fostering a culture where incidents including claims and complaints are seen as positive opportunities to learn lessons and improve services.
Ensure that systems and processes developed to enable the maternity service to comply with national guidance (NICE), legal and professional requirements and to achieve service excellence.
Coordinating with the health and Safety Department to ensure that risk assessments are undertaken and actions are carried out.
Support the assessment of clinical and non-clinical risk within the Maternity service and ensure that complaints, reported incidents and serious untoward incidents (SI’s) are investigated, managed and reported appropriately.
Support the clinical leads for Governance and Risk to ensure the Maternity Risk Management Group is fulfilling its role
Ensure SI’s are presented to the multidisciplinary Team.
Analyse risk information from Women Services and produce trend information to support mitigating action and wide learning from incidents, which have occurred.
Support the Women’s Service to reduce risk and further develop the positive culture of risk management.
Play a key role in embedding and integrating Maternity risk management structures, systems and processes by delivering tailored training programmes, assessment and investigation support.
Liaise with the practice development Lead to ensure that education and training to minimise clinical risk is available to clinical staff.
Actively work on own initiative but also be part of the trust clinical governance strategy.
Ensure that the Trust is fully aware of best practice and that risk management is integral to the operation of the Organisation.
Ensure that members of the Clinical governance Team and midwifery staff are kept appraised of contemporary risk management practice.

Experience
Essential criteria
- Significant experience in clinical midwifery practice
Desirable criteria
- Prior involvement in incident investigation, safety reviews, or governance processes.
Knowledge
Essential criteria
- Strong understanding of patient safety principles
- Excellent communication and leadership skills to influence practice and promote a safety culture.
Desirable criteria
- Experience in training or mentoring staff on safety-related topics.
- Skilled in clinical audit, root cause analysis, and quality improvement methodologies.
- Evidence of post-registration education in patient safety, risk management, or clinical governance is often desirable
