Main duties of the job

The post holder provides continuity of carer for women with complex health, social and safeguarding needs throughout pregnancy, birth and the postnatal period. Working within the Lotus Team, they coordinate personalised, trauma-informed care for women experiencing vulnerabilities including domestic abuse, mental ill health, substance misuse, homelessness, exploitation, social deprivation and safeguarding concerns.

Key responsibilities include managing a caseload of vulnerable women; undertaking holistic assessments and developing individualised care plans; coordinating care with maternity services and partner agencies including social care, housing, perinatal mental health, substance misuse and domestic abuse services; identifying and escalating safeguarding concerns; and attending multi-agency meetings including Child Protection Conferences, Strategy Meetings, MARACs and pre-birth planning meetings.

The role also involves advocating for women and families, promoting engagement with maternity services, delivering antenatal and postnatal care, supporting infant feeding, maintaining accurate records, contributing to audit and service improvement, and promoting safeguarding and trauma-informed practice.

The overall purpose of the role is to deliver compassionate, coordinated midwifery care that improves outcomes, reduces health inequalities and supports effective safeguarding through continuity of carer and multi-agency working.

Working for our organisation

Croydon Health Services NHS Trust provides hospital and community services across Croydon through a range of clinics and specialist centres. Formed in August 2010 from the integration of Croydon Community Health Services and Mayday Healthcare NHS Trust, we employ around 4,100 staff serving a diverse population of over 360,000 people.
