Job overview

Due to investment, we have a new role for a band 6 nurse within the Croydon Community stroke rehab team. The post holder will work collaboratively with the other professionals within the well established MDT, enabling people who have had a Stroke to maximise their independence in the Community. You will be supported by a highly experienced Band 7 nurse and provide nursing input to patients, especially with regards to blood pressure management, medication compliance, continence and skin integrity.

The team work Monday to Friday.

A full valid UK drivers license and vehicle for work are essential for this post.

Main duties of the job

Demonstrate clinical expertise by  delivering hands on care, using highly developed physical, analytical and problem-solving skills, in line with research and evidence based practice.

The post holder will liaise with the nursing staff on the Acute Stroke Unit at Croydon University Hospital and the local hyper acute stroke units to ensure appropriate and timely transfer of care into the Community. 

The post holder will be expected to work unsupervised in a variety of settings including the patient’s own home, Croydon University Hospital and other community settings as required.

To ensure that the care delivered to patients is a result of individual assessment of their need using an appropriate assessment framework and actively seek consent in the delivery of such care.

In conjunction with the MDT, be responsible for maintaining excellent standards of care for patients with multiple needs who require complex case management

EDUCATION AND QUALIFICATIONS

Essential criteria
  • Registered Nurse with current NMC Registration

KNOWLEDGE AND UNDERSTANDING

Essential criteria
  • Awareness of clinical guidelines in relation to Stroke
  • Evidence of sound understanding of role of nursing management of stroke

EXPERIENCE

Essential criteria
  • Recent experience as Band 5 Nurse working in Stroke in NHS
  • Experience working as part of an MDT
Desirable criteria
  • Band 6 nursing experience in stroke
  • Qualified nursing experience working in the community

SKILLS/ABILITIES

Essential criteria
