Job overview
A six month fixed term post to manage the Newham community neuro rehabilitation pathways for neurological and community stroke rehabilitation covering prevention, intermediate care, early and continuing rehabilitation and long term support (for up to 8 weeks)
The Newham Integrated Neuro Service works across several sectors and organisations, namely East London Foundation Trust (ELFT), London Borough of Newham (LBN), Community and the Voluntary Sector.
To lead with the National Service Framework for Neurological Disabilities. Implement NICE guidelines and to work towards the Stroke Clinical Guidelines 2024/2025.
Represent the services as required at a strategic and at North East London networks.
To hold a small clinical caseload consisting of highly complex neurological conditions.
To act as a Consultant Experienced specialist opinion) within the management of highly complex neuro/stroke service users.
Main duties of the job
Operational / professional management and overall clinical development of the Integrated Neuro-rehab Service.
To provide leadership within specialist area, developing strategy in response to national, North East London and local policy relevant to service areas
To contribute and support key stakeholders (e.g. Stroke Neuro NEL, ICB) in the development of the Neuro/Stroke rehabilitation pathways across North East London and contribute to NEL Neuro Rehabilitation Community Practice.
To contribute to the service effectiveness and be responsive to the changing needs of the local population.
To take responsibility for the delivery of comprehensive and high quality Specialist Community Neuro Rehabilitation Service.
To work closely with all staff across CHN in the management of all neurological conditions. To work collaboratively with multidisciplinary colleagues to provide integrated care for clients with neurological conditions. This includes working closely providing in reach to the acute sites and liaising closely with the Integrated Discharge Hub at Newham Hospital.
Represent the services as required at a strategic and NEL level
To hold a small clinical caseload consisting of highly complex neurological conditions at least 2 sessions per week.
To act as a Consultant (Experienced specialist opinion) within the management of highly complex neuro/stroke service users
To attend and contribute to the NEL stroke and Neuro meetings

Detailed job description and main responsibilities
To lead the MDT ensuring high quality of therapy intervention and coordinated approach to maximise rehab potential.
To negotiate with others around case management in complex cases
To ensure the implementation highly specialist programmes of care and treatment to meet the patient’s needs.
To adapt practice to meet individual patients’ circumstances, including due regard for cultural and linguistic differences
To ensure the team demonstrate high level of clinical effectiveness by use of evidence based practice and outcome measures, ensuring that these are embedded in the practice of all clinicians in the service area (to include PROMS/PREMS)
To provide professional support and advice to Specialist Neuro Therapies and to staff from other professional and organisational groups (e.g. nursing staff, social services staff) working with this care group.
To contribute and facilitate the development of specialist care protocols/packages to improve patient care using QI where possible
To contribute to working groups and policy development groups appropriate to service area and report findings and proposals to line manager
To attend trust senior lead meetings
To attend NEL meetings
To ensure that the MDT staff area are undertaking necessary levels of CPD and reflection in order to maintain professional registration with the AHP within a framework of audit.
To participate in workforce planning for the service in liaison with the service manager

Education/ Qualification/ Training
Essential criteria
- Degree or equivalent
- Evidence of continuing Professional development
- Health Professional/ Nursing qualification
Clinical experience
Essential criteria
- At least three years management experience within the NHS including:
- Staff management (including multi-professional staff management)
- Performance management
- Experience of managing change
- Experience of working with users and patient led organisations
- Experience of business case writing
- Experience of managing Health/Social Care in disability/Stroke or a related field
Skills and abilities
Essential criteria
- Good Understanding of Clinical Governance
- Good knowledge and understanding of national stroke policies and guidance
- Able to write and understand complex standard operating procedure
- Understanding and knowledge of stroke / Neuro issues
- Knowledge and understanding of how to improve and develop services
Experience
Essential criteria
- Previous leadership experience in Neuro
