Job overview

The Clinical Support Assistant will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The post holder will promote optimum independence and individualised client centred care to a delegated caseload.

 

The post holder will also support the Community Diabetes Team to deliver against key performance indicators, participate in structured diabetes education and community outreach and maintain quality of the service.

Main duties of the job

Main Duties and Responsibilities
•    The role includes supporting the Consultant, Diabetes Specialist Nurses, and the Specialist Dietician, ensuring the effective provision of nursing care to patients attending the integrated diabetes service-Enfeild
•    Part of the role includes delivery of Desmond Education Training programmes for people with type 2 diabetes. Training will be arranged through the Desmond National Training centre and support will be given to enable the successful applicant to achieve the required accreditation.
•    Successful candidate will need excellent clinical, organisational, IT and communication skills.
•    Part of the role will also include phlebotomy service, supporting patient with blood glucose monitoring using new available technology and devices with good understanding of results interpretation and management.
•    Part of the role will include vital signs monitoring, weight checks. Supporting clinicians with clinic preparation including blood test results.
•    The role also includes supporting the admin duties including booking and cancelations of appointments. Support with booking follow up appointments and reducing DNAs
•    The post holder will be responsible to support clinicians with auditing data when required.

Working for our organisation

The Integrated Diabetes Service is a nurse-led service supported by a multidisciplinary team, including a diabetes consultant, clinical nurse lead, diabetes specialist nurses, dietitian, psychologist, healthcare assistant, and a health and wellbeing coach.

The service delivers specialised care for adults with Type 1 and Type 2 diabetes, including support for pregnancy, renal, and preconception clinics. Care is provided through a fully integrated model, with close collaboration between community and acute teams.

The service adopts a multidisciplinary team (MDT) approach. Clinical activities include assessment, diagnosis, shared decision-making, and the delivery of personalised treatment and management plans, focusing on achieving the three treatment targets and completing the nine key diabetes care processes.

This includes comprehensive assessment of patient history and pathology, management of diabetes-related complications, and review of dietary and lifestyle interventions. The service also provides structured education programmes for individuals newly diagnosed with diabetes.

Detailed job description and main responsibilities

•    To contribute to the on-going assessment of a patient by completing the appropriate paperwork for allocated patients, recording and reporting all clinical and behavioural information to the nurse/therapist for action and planning purposes.
•    To undertake, and report back on delegated clinical activities that have previously been signed
competent. Eg: vital signs, routine urinalysis, glucose monitoring, diabetes education groups, increasing independence through activities of daily living.
•    To enable patients/carers to make most effective use of resources within the service by ensuring they receive up-to-date information sheets, responding to any questions constructively
•    To pass onto/seek assistance from other colleagues, promptly, any questions that you are unable to resolve
•    To liaise and refer patients to the MDT team as appropriate in accordance with local procedures.
•    To assist with errands and general duties appropriate to the clinical area as required.
•    To use clinical judgement and skills to report any untoward changes in carers or client’s health status
•    immediately to the relevant team member, DSN, GP, District Nurse or emergency services if appropriate.
•    To collect data from patients/carers for recognised outcome measure and document for therapists/nurses to interpret.
•    To provide care to patients with challenging behaviour by maintaining a calm safe environment and reporting back to the nurse/therapist as indicated.
•    To plan and implement straightforward tasks and work guided by standard operating procedures and protocols and undertake roles/duties as delegated by a nurse/therapist.
•    To undertake a variety of clinical intervention tasks after achieving appropriate competencies delegated by a nurse/therapist.

Educaion and qualification

Essential criteria
  • • GCSE grade A-C or equivalent in English and Maths ECDL certificate
  • • Administrative Training
  • • A recognised health course
  • • Willingness to undertake further training as appropriate
Desirable criteria
  • Higher Professional Diploma in Health and Wellbeing (NVQ/QCF – Level 3)

Experience

Essential criteria
  • • Experience of working in a health or social care setting
  • • Experience of working with people with long term condition
  • • Experience of working as part of a team
  • • Experience with the use of Microsoft Office and a variety of databases.

Skills

Essential criteria
  • • Knowledge of NHS Demonstrate an acceptable level of numeracy and literacy. An awareness of risk management.
