Job overview

We are an expanding cardiology team and we are looking for a dedicated cardiology nurse with experience of working in Cardiac Rehabilitation who is motivated to help shape the care we deliver.

Working closely with the Cardiology Lead Nurse and the wider Cardiac Rehab team, you will run and develop the Cardiac Rehabilitation service at the St Helier site, ensuring that optimum treatment and recovery pathways are in place for cardiac patients across the care continuum.

The successful candidate will aim to meet changing needs in this challenging environment.

Experience and achievement in nursing cardiac patients is a must for this role, as is an understanding of therapies and treatment. You must have a keen interest in health promotion and be able to encourage patients to achieve independence whilst simultaneously building a culture in the service that is progressive and innovative in its approach and outlook.

You will work with adherence to national guidance and will take the lead in ensuring processes are in place to support effective monitoring of outcomes and that the service is actively contributing to national and local audits. You will be responsible for development of the service and change implementation informed by continual monitoring. You will have a keen interest in Education, research, training and development.

Main duties of the job

The post holder will provide a high level of clinically effective, safe nursing intervention through assessment, planning of care, delivering and teaching management of disease for cardiac patients.

The overall aim is support patients to self-manage their ongoing rehabilitation, so reducing morbidity and premature mortality, by detecting and preventing complications. Under the supervision of a Cardiology Clinical Nurse Specialist or Consultant Cardiologist, the post holder will have the autonomy to deliver a specific plan of care demonstrating the skills and competency to meet the individual patient needs following a cardiac event or condition in which Cardiac Rehabilitation is required. The post holder will also be able to make informed decisions within their own scope of professional practice to ensure agreed standards, objectives and targets are met for cardiac patients

The post holder will be expected to help co-ordinate and organise an evolving and developing cardiology nursing service which optimises quality and continuity of cardiac health whilst aiming to prevent disease, promote health and educate regarding disease management.
