Job overview

Thurrock Integrated Community Team , North East London Foundation Trust is seeking an enthusiastic and motivated Registered Nurse with a Specialist Practitioner qualification to work in the Integrated Care Team  as Band 7 Complex Case Manager

You will need to be able to demonstrate the following essential criteria skills for the post:

  • Information Technology (IT) Skills
  • The ability to travel between sites
  • Non medical or community Nursing Prescribing 

 The post holder will be providing case management for a small caseload of highly complex adults on the ICT caseload with complex multi diagnosis, high use of statutory services and already receiving face to face care from the ICT.

 To improve outcomes for people with frailty and complex co-morbidities and reduce the need for these people to attend or be admitted into hospital or long term residential care.

. They will also be responsible for the delivery of safe care to a Users of service across the team, including being professionally and legally accountable. 

The post holder will line manage a team of Community Nurses and Health Care Support Workers. They will supervise and appraise staff members and students.

The post holder must possess excellent leadership and communication skills.

The post holder must hold a valid UK driving licence.

Main duties of the job

The post holder will be a Registered Nurse ideally with Specialist Practitioner qualification,  and significant experience of working within the community setting.  They will be required to hold experience and competence in all clinical skills offered in the community. They will be responsible for assessment, management, planning and delivery of care, including being professionally and legally accountable. 

The post holder will support their peers and locality lead and be an effective team member whilst working on their own initiative.  They will supervise staff members and students.  

The post holder will support the Operational Lead in the investigation and management of clinical incidents, complaints and maintaining safe staffing levels.

High Cost Area Supplement - Fringe

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,303 and a maximum payment of £2,198 per annum, pro rata for part time
