Job overview
An exciting opportunity to join a progressive and expanding Inflammatory Bowel Disease (IBD) team and an opportunity to help reshape the service.
The post holder will provide specialist nursing care, advice and support for people with inflammatory bowel disease. This will involve autonomously managing a case load of patients with Inflammatory bowel disease who are on biological and immunomodulator therapies in nurse led outpatient telephone clinics with Clinical Supervision from the senior Nursing team and Consultants. The post holder will undertake all relevant forms of nursing care and patient management, supported by evidence-based practice, liaising with the multi-disciplinary team, with the objective of providing high quality, safe nursing care. The post holder must always act in an accountable manner, adhering to the NMC Code of Professional Conduct and Trust polices.
Main duties of the job
- The post holder will be an integral member of the wider gastroenterology team consisting of specialist nurses and Doctors
- To deliver evidence based clinical care and review biologics patients and those on immunomodulator medications
- To help develop new ways of working with how we communicate with patients via the IBD adviceline
- To deliver MDT outcomes to patients via telephone and email
- To be self-directing and work autonomously, identifying and maintaining supportive networks.
- To demonstrate a higher level of practice through the provision of specialist nursing knowledge and expertise in the delivery of care through participating in local and regional MDT radiology/clinical/business meetings.
- To establish and maintain robust communication channels with professional colleagues across a range of services including primary, secondary and tertiary care.
- To support the IBD department in national audit and research initiatives.
- To take an active role in triaging patients care, promoting self-management and responding to helpline queries
- To rotate across sites within the Trust.
- There will be an opportunity to specialize in management of biologics patients.
