Job overview

We are looking for a motivated, skilled, and experienced Nurse to join our Community Mental Health Services in Greenwich Older Adult Team. The post is based at Memorial Hospital.

The post holder will be committed to improving the mental and physical well-being of mental health service users that are accessing care in our team.

Main duties of the job

The post holder will be responsible for a caseload of service users who have complex and substantial mental health needs. They will work as a member of the MDT ensuring that high quality individualised care is delivered and that this promotes recovery and maximise independence.

The post holder will be responsible for undertaking comprehensive assessment of service users and carers mental health and social care needs, in accordance with National CPA Guidance.

The post holder will be working in a supportive diverse team, where there are opportunities for personal development.
