Job overview
- To admit patients and ensuring that all patients are assessed on the ward preoperatively.
- To establish and maintain robust multi-disciplinary discharge planning arrangements in order to discharge patients safely and reduce length of stay, delayed transfers of care and re-admissions.
- To proactively co-ordinate, review and case manage complex discharges and to facilitate simple discharges by engaging the multi-disciplinary ward team.
- To ensure the discharge planning process is responsive to the needs of patients and their carers by means of joint working with the relevant acute, community and allied support staff.
Main duties of the job
- Undertake assessment of patients’ to be admitted on the ward preoperatively and pre-procedure and assessing their discharge needs in appointed area of responsibility without supervision.
- Co-ordinate and refer to appropriate therapy team if rehabilitation required.
- Arrange and Chair Multidisciplinary team meetings for individual patients with very complex discharge needs.
- Refer and liaise with Social Service Department and other local agencies e.g., ICT as appropriate.
- Work clinically on the ward and other areas where appropriate and as required, supporting the ward teams.

Shortlisting criteria
Essential criteria
- Registered Nurse (general)
- A minimum of 2 years post registration experience
- Venepuncture and cannulation
- Demonstrable knowledge of patient pathways and discharge processes
- Ability to assess, plan, implement and evaluate nursing care according to individual needs.
- Ability to effectively prioritise workload and to coordinate admission and complex discharges on the ward on a daily basis
- Ability to work independently and as part of a team and able to both influence and work across professional boundaries.
Desirable criteria
- Experience and understanding working in a vascular ward
- Experience in pre-assessment and discharge planning
Shortlisting

- Ability to take charge of the ward in the absence senior staff nurse.
