Are you a passionate, energetic Children’s Nurse?

An exciting opportunity has become available for a full time band 5 Community Children's Nurse to work within the Bexley and Greenwich Community Children’s Nursing Team and integrated Hospital at Home Team.

The team provides a seven day service from 08.00 to 20:30, there is also 24 hour on call provision for children & young people who require end of life care at home.

The team of specialist and generic children’s community nurses provide skilled, comprehensive nursing care to children and families with nursing needs in their own homes or community settings in the boroughs of Bexley and Greenwich. 

The Community Children’s Nursing and Hospital at Home Team has a competency framework to support the process of induction and professional development for the Children’s Community and Hospital at Home Nurse. After completion of this framework the post holder will share a caseload with a senior member of the nursing team and be able to work without direct supervision.

Main duties of the job

The CCN/HAH is part of an integrated network of services for children in Bexley and Greenwich providing highly skilled, nursing care to children and families with nursing needs in their own homes or community settings.

The post holder will be required provide a high standard of evidenced based nursing care for children and young people with the following conditions: acute and long term conditions, children with disabilities and complex conditions including those requiring palliative and end of life care.

To work in partnership with other community based statutory, voluntary organisations to deliver seamless services for children and their family.

 The post holder will be required to travel between sites and patients homes across the boroughs of Bexley and Greenwich.

Detailed job description and main responsibilities

You will work within an integrated Children's Nursing team covering both CCN/HAH, the role as a community Children's Nurse is highly varied. We are one integrated team covering both borough's of Greenwich & Bexley. The caseload is varied with referrals of CYP who have complex medical needs, long term health conditions and short term health conditions requiring rapid service input to enable CYP to be discharged from hospital early.

Education

Essential criteria
  • RSCN or RN (child) or Learning Disability Diploma / degree level
  • Current NMC registration
Desirable criteria
  • Evidence of continued professional development

Experience

Essential criteria
  • Knowledge of clinical skills in paediatric nursing
  • Experience of joint working together with other professionals
  • Experience in assessing children’s care needs, planning implementation and evaluation of care plans.
Desirable criteria
  • Ability to transfer clinical skills into the community setting

Skills/Abilities/Knowledge

Essential criteria
  • Demonstrate understanding of the role of the community children’s nurse
