Nurse Assessor

Band 6

Job overview

NHS South West London ICB is looking for registered nurses to join our All Age Continuing Care (AACC) team as Nurse Assessors. The team delivers the ICB's statutory responsibilities under the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.

You will assess adults in any setting, whether at home, in hospital or in a nursing home, to determine their eligibility for NHS Continuing Healthcare (CHC) or NHS-funded Nursing Care (FNC). Acting as nurse assessor and coordinator of the multidisciplinary team, you will establish eligibility and work with each person and their family to identify a suitable care package.

You will also review CHC and FNC funded clients in line with the National Framework (revised 2022), explaining the process and decisions clearly to people, families and professionals so that it is transparent and understood.

You will bring relevant nursing experience gained in a complex, multidisciplinary environment, ideally in mental health, learning disabilities or community nursing, an understanding of the National Framework, and excellent written and verbal communication skills.

Main duties of the job

The main duties of the role include:
•    Manage referrals from professionals, patients and carers, prioritising them and managing your own caseload.
•    Coordinate and take part in MDT assessments using national tools and within required timescales, determining eligibility for CHC and FNC in line with the National Framework.
•    Review CHC and FNC funded clients, and monitor the quality of commissioned care packages during care planning and reviews.
•    Keep accurate written records in line with the NMC Code, and share information appropriately while maintaining confidentiality.
•    Work across health and social care boundaries to gather specialist evidence and to inform and involve people, carers and professionals about the National Framework.
•    Call, attend or lead CHC and multidisciplinary meetings, including case conferences and discharge meetings, to make sure care is joined up across agencies.
•    Take part in complaint investigations and appeal responses, monitor care standards in local nursing homes, and raise any concerns about care delivery.
•    Provide reports and advice under safeguarding procedures for adults at risk.
•    Raise awareness of CHC among referrers such as GPs, consultants, care managers, discharge coordinators and the voluntary sector.
•    Take part in research, audit and clinical and peer supervision, and deputise for colleagues and Senior Nurse Assessors when needed.

Working for our organisation

The core purpose of SWL ICB is to improve the health of our population and ensure access to consistently high-quality services. Overseeing a budget of £4.3bn we hold accountability for ensuring the best use of the health budget for SW London’s population in order to improve health and healthcare, both now and in the future.

The core responsibilities of an ICB are to:

·       Improve outcomes in population health and healthcare.

·       Tackle inequalities in outcomes, experience and access.

·       Enhance productivity and value for money; and

·       Support broader social and economic development in their area

The ICB provides system leadership for population health, setting the evidence base and long-term population health strategy. We work as healthcare payers to deliver this, maximising the value that can be created from the available resources. This involves investing in, purchasing and evaluating the range of services and pathways required to ensure access to high quality care, in order to improve outcomes and reduce inequalities within South West London.
