Job overview
The Bolton Mental Health Directorate has an exciting opportunity for a dynamic and enthusiastic individual to apply for this role, to improve the service delivery within the adult Community Mental Health Team’s.
The successful candidate will play a central role in supporting the clinical and operational team to ensure high quality, recovery focussed care is delivered to our service users.
The post holder will hold a caseload and will lead assessment, formulation and prescribing of care / treatment planning, including discharge planning within agreed protocols and procedures and be an integral part of the Multi Disciplinary Team supporting clinical decision making and supporting the service pathways, developing nurse led clinics and providing brief interventions for people under the assessment pathway within the team.
The post holder will also support with providing a seamless pathway into & out of the CMHT, once interventions / goals have been achieved, to promote recovery and self-management, as clinically appropriate.
Main duties of the job
The post holder will provide advanced and expert level of practice (including non-medical prescribing) within the adult CMHT leading to improved patient outcomes.
The post holder will support the clinical and operational team to deliver on service improvements in the quality of care and safety through service development, evaluation, and audit.
They will be an integral part of the Multi Disciplinary Team supporting clinical decision making and supporting the service pathways, facilitating nurse led clinics and providing brief interventions. The post holder will also support with providing a seamless pathway into & out of the Community Mental Health Team, once interventions / goals have been achieved, to promote recovery and self-management, as clinically appropriate.
The Advanced Clinical Practitioner will hold a case load and will lead assessment, formulation and prescribing of care planning including discharge planning within agreed protocols and procedures.
The job holder may additionally lead on practice and service developments. The job holder is required to demonstrate clinical and practice expertise via interventions, supervision and modelling good practice.

Education/Qualification
Essential criteria
- • Registered First level nurse • MSc in Advanced Clinical Practice • Nurse Prescribing qualification
Desirable criteria
- • Clinical based Masters degree • Mentor Qualification
Experience
Essential criteria
- • Experience of working with clients with complex needs • Compiling protocols and clinical guidance • Audit • Experience of working at a senior clinical nurse level
Desirable criteria
- • Research • Experience of developing nurse led clinic • Evidence of autonomous practice • Experience in service development
Knowledge
Essential criteria
- • Advanced Clinical Practice skills • Management of service users with complex needs • Clinical examination skills • Accountability of own role and other roles within the team. • Knowledge of Local and national health policy • Extensive knowledge of psychological, social and biological factors and formulating these into a comprehensive bio-psychosocial formulation and clinical risk management plans. • Clinical supervision training and experience • Extensive knowledge and application in practice of leading complex adult safeguarding procedures, professional meetings, MHA, CPA, DOLs • Facilitate a culture of lifelong learning including involvement in the process of clinical supervision, AP student final sign off and appraisal. • Experience of coordinating & leading on a series of audits, action pans & projects
