A secondment opportunity has arisen for a Band 6 Discharge Facilitator at MFT (Wythenshawe Site).  The post holder will be expected to have specialist knowledge surrounding the care of patients requiring discharge from the hospital setting to either home or 24 hour care setting.  They will need to support colleagues in the effective management of patients in their specialist area who require a high level of judgement, discretion and decision making within an inpatient or outpatient setting.
