Out of Hours District Nursing Team Leader - Trafford

The post holder will hold the Community Specialist Practitioner Qualification and will be a core member of the nursing management team. They will work collaboratively to deliver high‑quality care across the Trafford Borough and support the development of a locally appropriate, integrated model of care delivery. The post holder will contribute to service improvement strategies and represent community nursing at neighbourhood, locality, and borough‑wide forums and groups.

Within this role, the post holder will have clinical responsibility for the assessment of patients, the development and implementation of care plans, and the delivery of nursing care. They will provide expert advice and guidance to the multidisciplinary team and ensure that all records and documentation are completed accurately and appropriately. The role carries overall 24‑hour responsibility for nursing care across Trafford, including operational management of the night service.

The 30-hour post includes 3 nights, there would be an expectation that some hours be utilised during the day for leadership and managerial responsibilities. The post holder will ensure that teams deliver a safe, effective, and high‑quality service at all times.

Liaises with:  Lead Nurse, citywide nursing team leaders, Specialist Nurses, AHP lead, Primary care, Social care, Human Resources Team, Quality Matron, Community Nursing Matron, LD Matron, Heads of Service, Education & Development Practitioners, Neighbourhood Teams, Governance officers,  Voluntary agencies
