Job overview

Interview -Weds 26/11/25, and afternoon of Fri 28/11/25. 

To work within a multi-disciplinary team to facilitate a range of recovery and abstinence focused interventions for adults presenting at accident and emergency in Stockport because of substance misuse related issues. The post holder will provide intense interventions for frequent attenders at A & E, offer rapid access to treatment for patients, monitor presentations at Stepping Hill Hospital, offer opportunistic brief intervention to patients, offer advice to staff on management of adults presenting at hospital on a frequent basis.

Main duties of the job

This will include undertaking comprehensive assessments of need, individual therapeutic interventions, recovery co-ordination and referral on to support services with a view to reducing overall attendance at A+E and subsequent in-patient admissions. The post holder will work primarily in the community service but also support Stepping Hill Hospital staff in the planned discharge of patients who have been admitted to medical and surgical wards.
The post holder will provide clinical interventions supporting sustained recovery within an ambitious, inspirational and outcome focused culture promoting hope, empowerment and choice.
The post holder will contribute to the services clinic responsibilities for alcohol and other substances (e.g. pre-detox bloods, support for community detoxification BBV screening, clinic room checks, medicines management commensurate with the grade).

Detailed job description and main responsibilities

To contribute to the provision and facilitation of a range of interventions focusing on managing individuals’ recovery capital and increasing their chances of achieving and sustaining abstinence. This will include psychosocial interventions, clinical interventions commensurate with the grade and under the supervision of Senior Nurses, provision of substitute medication where appropriate, support to access mutual aid and peer support networks, support to develop positive community relationships and support to access other services both substance misuse specific (e.g. rehab and community programmes) and mainstream (e.g. housing, employment, education and training.)

Education / Qualifications

Essential criteria
  • RGN / RMN (Including newly qualified and preceptorship Nurses for whom further training and support will be available)
  • Professional registration with NMC
Desirable criteria
  • Relevant training in specialist area
  • Training in sexual health/blood borne viruses.
  • Non-medical prescribing qualification or willingness to undertake this training.

Experience

Essential criteria
  • Experience of assessing clients’ health and social care needs in a drug or alcohol setting.
  • The provision of advice and support on health and/or social issues.
  • Experience of working with individuals who have drug and/or alcohol problems
  • Experience of providing support to vulnerable/socially excluded individuals/groups
  • Experience of working as part of a team to achieve a shared vision/goal
Desirable criteria
  • Experience of community engagement and service user and carer involvement
  • Experience of group programme design and facilitation

Knowledge

Essential criteria
  • Knowledge of the physical, psychological and health effects of drug and alcohol use and appropriate interventions
  • Experience of multidisciplinary working.
  • Experience of working with clients on a one to one and/or group work.
  • Ability to acknowledge diversity and promote antidiscriminatory practice / equal opportunities
  • Knowledge of issues relating to Drug and Alcohol and understanding of the principles of harm minimisation
  • Knowledge of the importance of mutual aid and peer support networks

Contribute to the Service’s clinical and nursing duties including taking of pre-detox bloods, support for community detoxification clinic room checks, medicines checks, administering of agreed medicines suitable for the client group in line with the expectations of the service Consultant in Additions and the Trust’s Chief Pharmacist
Provide advice, education and support to service users, families, carers and wider communities on public health issues relating to substance use and safety, prevention and health promotion.

To provide a range of services within a harm reduction model, including reducing alcohol related harm, safer injecting, sexual health and general health advice.

To provide general health assessment, monitoring and interventions (e.g. blood pressure monitoring, lung function testing) whilst enabling the service user to access mainstream community health services.

Provide specialist clinical information, support and advice to other health and social care professionals and internal and external agencies.

Support local public health campaigns and engage in public facing substance misuse and health messages.

Engage service users in therapeutic alliances and facilitate up take of wider treatment and recovery services.

Enable Service Users to access Blood Borne Virus screening and vaccinations for hepatitis A and B and where appropriate Flu vaccination and where indicated facilitate access to these services for the User's family members.

Facilitate and evaluate group work programmes and support the visible recovery community including, for example, Preparation for Deox.

Provide verbal and written information to ensure service users can make informed choices about their recovery journey.

Work with service users within local community amenities to maximise individual health and wellbeing potential whilst promoting social networks and active citizenship.

Actively engage with carers and significant others both in advising their own needs and where appropriate as part of the service users care.

Actively engage with the wider community as part of the services undertaking in order to forge positive recovery networks and develop community capacity enabling service users to build positive social networks.

Actively engage with other agencies both across the borough and wider, liaising on behalf of service users.

Work with the Integrated Children and Family Service in relation to the Boroughs Supporting Families initiatives.

Provide reports where required in relation to Child and Adult safeguarding proceedings and attend Child In Need and other safeguarding meetings.

Maintain knowledge of local and national criminal justice agenda and other emerging priorities and incorporate this into practice within the Service framework.

Ensure effective data collection and information provision including the use of computer-based systems.

Make and maintain clear and concise note records compliant with Trust expectations.

Liaise with multi-disciplinary teams seeking support and advice as necessary.

Assist in the development of services where needed.

Participate in training and other opportunities for the professional development of self and others, including the supervision of more junior staff or students.
