Job overview

This is an exciting time to join the team at  Stockport drug and alcohol service.  We provide a non-discriminatory and user friendly service, offering recovery focused interventions to reduce substance misuse related harm for individuals, their families, and the local community. We work in partnership with wider health and wellbeing services to meet the diverse needs of Stockport residents, and to deliver a high standard of care. 

All new starters to Pennine Care NHS Foundation Trust who are appointed to a post which requires a Disclosure Barring Certificate check (DBS) will be recharged for the full cost of the check. Substantive staff will have the amount deducted from their first months’ salary.

Main duties of the job

The key remit of this post is to identify people and work more intensively with clients with co-occurring conditions, and also to support the wider workforce around this area. The post will take direct referrals from mental health wards and support discharge plans. Follow up and community engagement with treatment (for Drug and Alcohol, not mental health) is undertaken by the post holder who will carry a reduced caseload.                         

  • To actively manage the recovery journeys of a  reduced case load of drug and alcohol clients with complex needs.
  • To actively manage clients receiving OST
  • To support both drug and alcohol clients through community and in-patient detoxification programs.
  • To provide comprehensive initial and on-going assessment of need.
  • To formulate, implement and review care plans.
  • To support group work programs.
  • To actively engage with carers and significant others both in advising their own needs, and where appropriate as part of the service users care.
  • To provide reports in relation to Child and Adult Safeguarding and attend Child In Need and other safeguarding meetings

Education / Qualifications

Essential criteria
  • Level 3 training in Community Health or Drug and Alcohol Use Practitioner course)
  • Ability to use information technology systems
  • Evidence of or willingness to work towards recognised substance misuse qualification.
  • Short courses and experience to graduate level
Desirable criteria
  • Relevant training in specialist area
  • Training in sexual health/blood borne viruses.
  • Relevant Professional qualification e.g. RGN / RMN / DipSW./ Substance misuse Practitioners certificate

Experience

Essential criteria
  • Experience of assessing clients’ health and social care needs in a drug or alcohol setting.
  • The provision of advice and support on health and/or social issues.
  • Experience of working with individuals who have drug and/or alcohol problems
  • Experience of providing support to vulnerable/socially excluded individuals/groups
  • Experience of working as part of a team in order to achieve a shared vision/goal
Desirable criteria
  • Experience of community engagement and service user and carer involvement
  • Experience of group programme design and facilitation
  • Experience of working with mental health issues

Knowledge

Essential criteria
  • Knowledge of the physical, psychological, emotional, and mental health effects of drug and alcohol use and appropriate interventions.

  • Ability to acknowledge diversity and promote anti-discriminatory practice / equal opportunities
  • Knowledge of issues relating to drug, alcohol and mental health and an understanding of the principles of harm minimisation
