Job overview

Stockport Foundation Trust is seeking a highly motivated diabetes nurse to join their Integrated Diabetes Team. We at Stockport take pride with the understanding that the team manage all aspects of the speciality including Inpatient work, pregnancy, insulin pumps, young adults and transition, complex wound and foot alongside community referrals from primary care. The post advertised requires minimum of 2 years experience working as a DSN with knowledge of diabetes technology and implementation. 

Main duties of the job

  • To assess, plan, prioritise, implement and evaluate nursing interventions to meet the patients’ needs which may be in the community or acute setting.
  • To actively participate in delivering health care for people with diabetes in Stockport; to ensure high quality and safe standards of care.
  • To advise, educate and support clients and carers on the management of diabetes by providing specialised programmes of care in relation to health and illness and the development of self-management techniques including individual and group sessions
  • Adhere to Nursing & Midwifery Council (NMC) Code of Conduct and associated guidelines.
  • Maintain confidentiality in all aspects of care and employment

  • Be actively involved in clinical supervision of nursing and midwifery

staff.

  • Act as a role model at all times.
  • Maintain relevant professional registration.

Detailed job description and main responsibilities

·   The post holder will be expected to work as part of the integrated specialist diabetes team delivering diabetes care to adults in both in-patient and community settings

Work as part of the MDT in specialist clinics such as ante-natal, pump, young adults and inpatient diabetes cover.

·   Provide specialist expertise, knowledge, advice and education demonstrating a sound understanding of the management of patients in this area

·  Demonstrate a high level of judgement, discretion and decision making in diabetes management within the acute or community setting, with an aim to reduce hospital attendance/admission and reduce length of stay during in-patient care

· Deliver education including structured programmes to people with diabetes to enable them to self-manage their condition

· Develop and deliver educational/training programmes and act as a specialist resource for staff and colleagues to improve standards of diabetes care

· Develop and implement diabetes policies and standards based on national guidelines, as well as provision of specialist care

· Monitor and improve standards of care through supervision of practice, clinical audit, evidence based practice, learning and development and the provision of skilled clinical leadership

· Support the service lead with service improvement development

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Education & Qualifications

Essential criteria
  • 1st level registered Nurse - NMC
  • Nursing Degree/diploma level or equivalent experience.
  • Teaching qualification or equivalent
  • Non medical prescriber or willing to undertake
Desirable criteria
  • Demonstrate effective teaching and presentation skills

Knowledge

Essential criteria
  • Evidence of proactive involvement in the mentorship and development of nurses and nursing practice
  • Demonstrate initiative and innovative practice Ability to work autonomously and within a team
  • Knowledge of diabetes specialist clinical treatment and technology
  • Ability to develop leading services and change practice
  • Working within an acute and community setting
Desirable criteria
  • Ability to implement research/undertake audit recommendations

Experience

Essential criteria
  • Previous experience of working as a diabetes specialist nurse
  • Ability to demonstrate evidence of working in a multi-disciplinary team
  • Ability to demonstrate evidence of change management

Skills & Abilities

  • Excellent time management/priority setting skills
  • Excellent verbal and written communication skills
  • Evidence of professional/clinical knowledge in diabetes or experience of working as a Diabetes Specialist Nurse
