Job overview

Band 6 District Nurse- 37.50 Hour posts  District Nursing

In the absence of the Band 7 Team leader, the Band 6 District Nurse will be responsible for leading the neighbourhood community team contributing to the development and the delivery of high quality clinical nursing care, maintaining and improving professional standards within the community setting of patients own homes. The post holder will be responsible for assessing the needs of patients and planning interventions to alleviate symptoms prevent complications of acute illness and to prevent unnecessary hospital admission. You will work autonomously whilst also having regular support/ communications with your band 7 Team Leader. In addition duties will include staff rostering, 1-1 meetings, staff appraisals and monitoring training metrics to ensure a high standard of care is delivered.

Main duties of the job

 

We are looking for flexible, innovative practitioners’ with a variety of clinical skills. The District Nursing Service provides care in patients’ homes as well as residential homes caring for patients with complex needs and supporting carers.

Stockport District Nursing Service offers a supportive preceptorship programme for staff new to community and welcomes applications from newly qualified District Nurses, returning to District Nursing or existing post holders.

The post holder should have good organisational and assessment skills be flexible and wiling to cover the service between the hours of 08.00hrs-18:00  7 days per week.

Detailed job description and main responsibilities

This is an exciting opportunity to join the integrated community service as a Community Specialist Practitioner Band 6 with the Stockport District Nursing Service with Stockport NHS Foundation Trust.

As a specialist practitioner you will be able to demonstrate effective leadership skills, ability to problem solve and illustrate a high level of clinical expertise in supporting staff in practice. You must be able to display effective communication skills with patients, family, carers as well as the wider multi-disciplinary service.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust’s policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary 

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.

Essential criteria
  • 1st level registered nurse
  • Degree / Post Graduate Diploma in District Nursing ( Community Specialist Practitioner)
Desirable criteria
  • Nurse Prescriber V100/V150/V300

Knowledge

  • Up to date knowledge of nursing issues
