This is a fantastic opportunity to be part of the Wigan referral and assessment team. 

We are looking for skilled practitioners, with mental health experience, who are able to complete triage and assessments at the front door service into secondary care mental health. 

Successful candidates will play an important part in the in the delivery of high-quality service provision.

As part of the team, you will be empowered to shape and build the service as community transformation develops in Wigan. You will help build and maintain positive working relationships with colleagues and the wider network. 

Prif ddyletswyddau'r swydd

As a nurse practitioner within the R&A hub you will - 

Provide clinical provision of rapid assessment and care/pathway planning for people referred into the R&A presenting with mental health problems. 

The post holder will be expected to carry out mental health assessments, accessing all available sources of information and including, where appropriate, the views of family and carers when relevant.   

To undertake and record comprehensive need/risk assessments, developing clear goals and timeframes for the onward interventions with the service users.

  • Experience of undertaking initial biopsychosocial assessments.
  • Evidence of post registration training/development.

Knowledge

  • Independently undertake initial mental health assessments identifying a service users’ needs and plan goal focused onward care.
  • Complete risk assessments which reflect this risk a service user may pose.
  • Transfer the risk assessment into robust risk management and pathway planning.
  • Effective communication with service users, carers and members of the multidisciplinary teams.
  • Understand the care pathways between primary care services and secondary care services.
