In collaboration with Dementia UK, Mersey care NHS Foundation Trust are looking to recruit an exceptional Band 6 registered nurse with a passion for, and experience of, providing relationship centred dementia care to join our Admiral Nurse Service. Work base will be predominately Halton however there is an expectation of cross cover with Knowsley team if needed. 

Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person living with dementia with complex needs.

As an Admiral Nurse, you will deliver specialist one -to-one support, guidance and practical solutions for families, alongside supporting best practice and providing role modelling within Merseycare NHS Foundation Trust.

Provide specialist nursing assessment, using the Admiral Nurse Assessment Framework or appropriate tools, for families affected by dementia, including positive risk taking, managing distressed behaviours and risk minimisation strategies

Provide a range of bio-psychosocial interventions to promote health and wellbeing for people affected by dementia and their families

Act as an advocate for people with dementia and their families to promote their rights and best interests

Support and advise on carer assessments and reviews

Work collaboratively with other professionals to improve the identification and management of co morbidities and frailty for people with dementia and their families

Develop and support care plans for people with dementia and families including positive risk management

Support people with dementia, families and staff to understand and respond to changes in behaviour and relationships as a result of dementia

Ensure the safeguarding of vulnerable adults and that all legal requirements relating to Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), and Liberty Protection Safeguards (2020) are considered and adhered to in relation to patient care

Work collaboratively with health, social and voluntary sector services to achieve coordinated care

Work in partnership with colleagues to promote the use of advance care planning for people with dementia and to support and inform families around advance care planning, best interest decisions and end of life care

Refer to secondary care if there is deterioration in condition/a review of medication including antipsychotics is required or any other concerns regarding a persons health that cannot be managed in primary care, in line with their dementia expertise

The successful applicant will be an NMC registered nurse, with significant post-registration clinical experience of working with people with dementia and supporting their carers/supporters, along with formal knowledge of dementia acquired through clinical post-graduate training/education.

You will receive ongoing support from Dementia UK to maintain and strengthen your clinical expertise as a specialist nurse within dementia care. You will also become part of the wider network of Admiral Nurses and will have the opportunity to access relevant post graduate courses and develop professionally, as informed by the Admiral Nurse Competency Framework.

Knowledge

  • Demonstrable experience in relevant area of specialised nursing practice or equivalent
  • Awareness of current national and local agenda in health and social care
