We’re looking for an enthusiastic and dynamic Discharge Liaison Team Leader to join our Integrated Discharge Service and play a key role in shaping the future of patient flow within our organisation.

As the Team Leader, you’ll hold a pivotal management position—providing leadership, direction, and professional support to the Discharge Liaison Team. You will lead from the front, ensuring discharges are completed safely, effectively, and in line with the Optimal Hospital Flow Framework (OHFF).

You’ll coordinate the daily activity of the discharge team, drive collaboration across wards, community services, and partner agencies, and lead initiatives that reduce length of stay and prevent avoidable delays. This is a role where your ability to problem-solve, influence, and inspire others will have a visible impact on both patient outcomes and system flow.

As a senior clinician, you’ll act as a role model for best practice—promoting professional standards, supporting staff development, and ensuring compliance with OHFF and governance frameworks. You’ll also deputise for the Senior Nurse for Discharge when required, providing operational oversight and assurance across the organisation.

 

Main duties of the job

  • The Discharge Liaison Team Lead holds a key management position, providing leadership and direction to ensure timely, safe, and effective patient discharges in line with the Optimal Hospital Flow Framework (OHFF). 

  • The post holder will lead the discharge team in coordinating and implementing effective discharge planning, minimising delays, and reducing patients’ length of stay through proactive management and collaborative working across multidisciplinary teams and external partners. 

  • They will act as a role model, promoting best practice and supporting the professional development of team members, while maintaining a focus on effective timely discharges, Patient experience, and OHFF compliance. 

  • The post holder will also deputise for the Senior Nurse for Discharge as required, providing operational oversight, assurance, and leadership to maintain safe effective discharge planning and discharge performance across the organisation. 

  • The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

  • Qualified RGN Educated to Masters level, working towards or possess a demonstrable equivalent level of knowledge and experience
