Job overview

Harrogate Hospital has an exciting opportunity for an experience nurse to join the Children's Outpatient Department as Paediatric Transition Diabetes Specialist Nurse.

The Paediatric Outpatient Department and Children’s Diagnostic Centre delivers high quality, safe and effective care for children and their families and carers. It offers a wide range of consultant, nurse and allied health profession clinics. The department’s function is to see, assess and treat patients with various conditions in a timely manner taking into consideration the patient’s individual requirements. The Paediatric Outpatient Department is also a source of advice and expertise for others in the Trust who may work with children e.g. ENT Outpatients, Emergency Department, Dermatology, Dentistry.

Main duties of the job

·       To scope and design the clinical transition pathway for young people aged 16-19years embarking on transition to adult services along with the MDT members.

·       To provide specialist and advanced advice and support to multi-professional teams to develop the transition pathway and improve the service provided

·       Support and facilitate the transition strategy for diabetes with medical, nursing and other involved staff groups across the trust to ensure that young people aged 16-19years who have diabetes have a positive experience throughout the service.

·       Ensure all young people who have diabetes have a clearly defined pathway of transition, ensuring a supportive, smooth and seamless transition to adult services

·       To demonstrate improved clinical outcomes for young people with diabetes by developing best practice to ensure that young people’s specific needs are met, to reduce variation in service delivery and reduce risk for young people transferring  between paediatric and adult services.

  • In collaboration with the paediatric diabetes MDT, actively advocate for young people in their preparation for transition and during the transition period to adult services.

Criteria

Essential criteria
  • RSCN/Children’s Nurse
  • Postgraduate Paediatric Diabetes Degree
  • Postgraduate non-medical prescribing
  • Evidence of on-going professional development in area of specialism
  • Substantial experience in paediatric diabetes care as a PDSN
  • Previous experience in a leadership role
  • Experience working within the community
  • Experience in safeguarding children.
Desirable criteria
  • Quality Improvement Knowledge
  • Regular attendance at quality-of-care meetings/governance meetings.
  • Experience of transition services (paediatric to adult care)
  • Regular attendee at national level focus groups for children with diabetes.
