Macmillan Personalised Care Transformation Lead

Job overview
This post requires a well organised, capable health or social care professional with experience in project management and / or quality improvement initiatives? It is a great opportunity for someone with a good understanding of cancer services, experience with quality improvement and project management skills to bring together the Personalised Care and Support (PCS) Programme within Kettering Hospital and work in collaboration with counterpart across the Group model.
The successful candidate will work closely with the Head of Nursing for Cancer, the Macmillan Cancer Information and Support Service lead and clinical teams to deliver the Programme and Personalised care strategy outlined in the NHS England Long Term Plan for Cancer. Elements of the programme include Holistic Needs Assessments (HNA), End of Treatments Summaries, Health & Well being events (HWBE) and Personalised Care Follow Up Pathways with remote monitoring systems.
The role will include scoping and mapping of the existing cancer workforce's capability to deliver the Macmillan's Right by You Principles, and responsible for ensuring their understanding of this as a way of working, engaging with and influencing healthcare professionals, with a project management function to ensure the smooth delivery of the Personalised care agenda in the Trust.
Main duties of the job
Key activities and outcomes of the post are;
- To lead and project manage the Personalised Cancer care programme of work and agenda work across the Trust
- To embed an effective process for completion of holistic needs assessment and care planning,
- To as embed treatment summaries, stratified follow up for specific tumour groups
- To assist and support in the creation of tumour site specific Health & wellbeing events and programmes.
-Embed Macmillan's 'Right By You' principles as an ongoing culture change
-Scope existing services, mapping gaps and identify priority areas specifically relating to existing processes.
-Work with IT staff to support new ways of working and ensuring the development of Somerset reflects the needs of the Personalised Cancer care agenda.
- Work with the Head of Nursing for Cancer, the Macmillan Cancer Information and Support Service Lead, the Cancer Clinical Nurse Specialist's, Cancer Support workers, Macmillan Psychologists to embed HNA's and contribute to the development of Health & Well being events
- Development a cancer patient 'lived experience' group and accountability for the group.
- Regular networking and working with internal and external stakeholders including representatives from the integrated acre board and East Midlands Cancer Alliance
- Presentation of project updates, written reports and data collection.

Person specification
Qualifications
Essential criteria
- Registered Nurse/AHP
- Master’s Degree or relevant level of training
- Qualification in programme management or equivalent level of education or training
Desirable criteria
- ‘HOPE’ Facilitator or willingness to undertake training to acquire ‘HOPE’ facilitator competency
Knowledge/Experience
Essential criteria
- In-depth knowledge about the stratification of cancer pathways including the key elements outlined in the national personalised care and support agenda
- National frameworks related to cancer care
- Considerable experience of program management
Desirable criteria
- Experience in managing service transformation /change

You must have appropriate UK professional registration.
