Job overview
An exciting opportunity has arisen to join the Corporate Complex Discharge Team as a Discharge Co-ordinator.
The Complex Discharge Team provides support for patients who have been identified as needing support from partner organisations for a safe and timely discharge.
The ideal candidate will have excellent people, planning and organisational skills and be able to work under pressure. You will also need to be highly motivated and flexible to work over a 7 day period.
The role will focus on supporting our patients with discharge plans to avoid admission to hospital or to commence planning of discharge care needs from point of admission and you will be supported by a wider Corporate Discharge Team and Ward Clinical Teams.
You will need to demonstrate proactive, enthusiastic, energetic patient centered care planning.
Main duties of the job
Working in collaboration with the multidisciplinary team, the post holder aims to facilitate effective, efficient and safe discharges through admission avoidance in urgent care areas and capture newly admitted patients with complex discharge needs commencing discharge planning through SBAR assessments from the point of admission within Kettering General Hospital. As an enthusiastic and adaptable individual, be able to work proactively whilst championing and promoting #HomeFirst philosophy and supporting the clinical and non-clinical workforce. To have an understanding of the internal escalation process, and how effective discharges can help ensure a proactive and effective organisational response to demand and pressures.
Expected to deliver care and services within the strategic objectives of the organisation and promote the principles of excellent discharge planning and delivery. These principles will recognise the need to promote safe practice; value the beliefs and aims of service users and carers.

Qualifications
Essential criteria
- Educated to GCSE level or equivalent level of education, training or experience in a healthcare setting
- Knowledgeable about health and care issues
- NVQ 4 or hold a foundation degree or similar experience in healthcare
- GCSE (or equivalent) passes in Maths and English
Desirable criteria
- Practical knowledge of the discharge process
Skills
Essential criteria
- Able to communicate information effectively in verbal, electronic or written formats to staff, patients, relatives and various agencies
- Well-developed organisational skills
- High standard of IT skills with the ability to input and retrieve information with accurate data entry.
Desirable criteria
- Able to deal with ‘challenging’ situations, including anger / aggression from patients / relatives / x 8 complainants
Applicant requirements
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
