 We are looking for a experienced RNLD or RMN to manage the day to day running of the Primary care liaison Team based in the City and County of Nottingham and the Acute Liaison Team based in NUH Hospitals.  The post holder’s key role is to lead the primary care team to support adults with intellectual disabilities to access mainstream health services.  The Post holder will lead on the development GP registers who are identified as having a learning disability registered with a GP in Nottingham City and Nottinghamshire.

 

The Role includes also the Leadership and Management of the Acute liaison Nurses based within NUH Hospitals and will involve leading the team to supporting patients with learning disabilities and their families to navigate the hospital journey. Reducing health inequalities.

 

Main duties of the job

·         To provide effective leadership to all staff within allocated team.

·         To demonstrate clinical leadership through personal practice.

·         To manage and monitor daily staffing levels and skill mix to comply with agreed staffing levels, resource  to provide an effective and safe service.

·         To be responsible for recruitment and selection, performance management, sickness absence, appraisal and personal development, grievance and disciplinary issues in accordance with HR policies.

·         Identify Learning and development needs of staff and facilitate the required training.

·         Provide clinical advice on complex issues and referrals to members of the team.

·         Work collaboratively with the care team in facilitating timely assessments within the service.

·         Ensure care is delivered within evidence of best practice.
