Are you looking for a change in direction or a new challenge? if so an exciting opportunity has arisen for a Clinical Nurse Specialist to join our Haemostasis & Thrombosis service.  we are a close and dynamic team that prides itself on patient care and team work.  The service plays an integral role in the Trust's vision for the future and provides an exciting opportunity to develop in this area of expertise.

You will work alongside an established team of Clinical Nurse Specialists to deliver high quality patient care and support for health care professionals  

Main duties of the job

  • Management of anticoagulation for patients on vitamin K antagonists
  • Running a morning clinic Monday - Friday at QMC campus
  • Providing a ward discharge service for patients on vitamin K antagonists across both City and QMC campus.
  • Arranging appropriate Anticoagulation for patients referred post USS scan with Deep vein thrombosis 
  • Providing support and telephone counselling to patients who are discharged following a Pulmonary Embolism
  • Offering a patient self-testing training service using near patient INR testing

The Anticoagulation team is a small established team of approx 16 staff including clinical nurse specialist , pharmacist and clerical staff 

The team are hardworking, enthusiastic, dynamic and committed to providing outstanding patient care.  Service development is ongoing  within anticoagulation and we embrace change to improve the care we provide.  we take part in regular audits and we are currently taking part in a national research study.  we have recently had some of our work published nationally and also by NICE

Training & Qualifications

Essential criteria
  • Current level one NMC registration Relevant post-basic study e.g. specialist pathway Continuing professional development/ reflective profile

Desirable criteria
  • Diploma or degree in health/nursing related studies
  • Formal leadership and management training

Experience

Essential criteria
  • Relevant experience in an acute setting
  • Supervision of non registered staff, pre registration and post basic studies
  • Ability to convey sensitive information in an empathetic manner to patients, relatives/carers and staff
Desirable criteria
  • Demonstrate an involvement in link nurse or project work
  • Audit or research within the clinical area
  • successful implementation and management of a nursing change process

Communication and Relationship skills

Essential criteria
  • Demonstration of team working and ability to work within a team
  • Demonstrate effective written and verbal communication skills
  • Produce timely, accurate written/electronic records and documents
  • Demonstrate basic IT skills
Desirable criteria
  • Demonstrate evidence of provising formal and informal teaching sessions

Analytical and judgement skills

  • Demonstrate an ability to assess, plan, deliver, prioritise and evaluate patient care
